Recent industry data shows that a large majority of organizations report using AI in at least one business function, and many manufacturers recognize AI as critical to competitive advantage.

However, moving beyond experimentation to meaningful, scaled results remains rare. While adoption is widespread, only a fraction of pilots are fully integrated into core workflows and driving measurable outcomes. That gap isn’t because the technology can’t produce insights; it’s because companies are still wrestling with how to operationalize AI at scale and confidently manage its outputs when things don’t go as planned.

When AI gets it wrong, who owns the outcome?
